According to the document "By memorandum dated October 16, 1959, the Administrator instructed the Associate Administrator to establish a formal, NASA-wide Reliability Program. A number of discussions of the subject at Headquarters have occurred recently, including a fairly extensive one at the first meeting of the Space Exploration Program Council held on February 10 and 11, 1960. The purpose of the following remarks is to outline, the current state of our plans in this connection."
